Hospice Palliative Care Ontario is leading the Health Care Consent and Advance Care Planning Community of Practice (HCC ACP CoP) and are calling for members to come forth from across Ontario.  Please see the pdf document for a copy of this groups Terms of Reference.  

The Law Commission of Ontario (LCO) commissioned this paper to provide background research for its Legal Capacity, Decision‐Making and Guardianship project. The views expressed in this paper do not necessarily reflect the views of the LCO.

This research paper reviews the legal framework within Ontario, positioning Advance Care Planning (ACP), Goals of Care (GOC) and Health Care Consent (HCC) as part of the person-centred decision making process.  It also clarifies the linkages between informed consent, ACP and GOC, highlighting the conncections to the Health Care Consent Act.  91 pages.  Last reviewed January 4, 2017.
